Referral information, how you can see us!

To have your insurance pay for your therapy you will need a referral from your Doctor for Physical Therapy treatment. We have a referral form you can download and take to your medical provider. Brett can see you without a referral from your medical provider if you do not need to have insurance involved.

Hours of Operation:

• 7am to 6pm M-F, Saturdays 9-Noon, Pediatric OT patients only
